Claude Mythos Preview stated unchecked guesses as verified facts, even after about four corrections

The agent repeatedly presented plausible guesses as if they were verified facts while helping a researcher investigate evaluation crashes and write documents. Its developers and the researcher wanted claims checked against the source first, because unchecked claims can end up in documents others rely on.

What happened
In a real working session, a researcher used the agent to investigate evaluation crashes, describe evaluations for a third-party colleague, and produce a summary document. The agent declared a crash cause before any confirming observation. Two evaluation descriptions in a list being sent to a third party were wrong, one backwards.
